There
are three centres that provide Age Concern’s home care and
day care in Manchester, in the North at Miles Platting, in Central
at Gorton, and in the South at Crossacres in Wythenshawe.
All three centres provide information, advice and support to older
people with higher support needs, to their families, their carers
and to fellow professionals.
Although the home care and day care services are available separately,
the purpose of the service is to provide the support network that
will make it possible for older people to live longer in their own
homes. It is possible to combine home care and day
care as a co-ordinated package where required.
Day Care
Our Care Centres offer a stimulating and enjoyable day out for older people who would otherwise have to stop at home all day. Activities are organised within the centres with occasional trips out for those who want them. A small daily attendance charge is made for these services.
The day centres are open all year round, Mondays to Fridays. Transport to and from the centres is provided as well as a healthy lunch and refreshments.
Sometimes Adult Social Care at Manchester City Council or the Mental health Joint Commissioning Executive will contribute towards costs. Normally day care is available as the result of a referral by the
Manchester Mental Health and Social Care Trust, by the Social Services
Department, by a GP, or often a combination of these agencies.

Support at Home
All three of our Care Centres also have a team of home support workers who provide care in people's own homes.
Home care is normally provided on a regular basis, seven days a week, following a home visit and a detailed assessment of the client's needs.
This service is available during evenings and weekends as well as during the day.
The home support team can also provide personal assistants for a special trip out.

Home support can be arranged directly and is sometimes arranged through the Adult Social Care at Manchester City Council who can contribute towards the costs. All of Age Concern's free services such as benefits advice, advocacy and so on are available to home support clients.
